It can be hard to talk to parents about their child's weight. This letter presents the issue as a concern for their child's health. It is easier for parents to hear, "I am concerned about your child's health," as apposed to, "Your kid needs to loose weight." It works well to send the BiW4Teens Program Brochure home with this BMI letter.
This brochure explains why a high BMI is unhealthy and briefly describes primary causes of a high BMI. Students and parents learn about program content, how the program works, and given the website address to order the program.
This health habits assessment helps youth identify their own unhealthy habits which are contributing to their high BMI. It is also useful information to you, the school nurse. Youth can find this habits assessment in program session 5 of the BiW4Teens workbook.
Use the Habits Explanation Cue Card to talk youth about how their unhealthy habits have a negative impact on their energy balance.
Youth who work with an accountability coach experience much higher success rates of program completion. Ninety-five percent of youth who complete the program experience multiple health benefits in addition to lowering their high BMI. School nurses make excellent accountability coaches. This form will help you, the school nurse, monitor the student's program progress. Page 2 has a list of program sessions that you can check off as youth complete them. Page 3 lists healthy habits - check off the ones youth are working on. Pages 4 -7 list learning objectives for each program session, which provide great conversation starters when talking to youth about program sessions they have completed.
This care plan is available for school nurses who are required to or want to have a care plan for youth working on lowering their high BMI.
This log is for school nurses who have several youth using the Biw4Teens program. This is a great tool to help busy school nurses keep track of all the youth who are working with them to lower their high BMIs.
This written contract strengthens a student's commitment to follow through and finish the program. It also clarifies the partnership between themselves and the school nurse, their personal accountability coach.
This contract serves the same purpose as the Program Commitment Contract for Student - School Nurse. It includes the parent in the contract agreement. It is intended that only one of the contracts be used. If the parent wants to make a commitment with the school nurse, this is the contract that would be used.
Use this letter to inform the student's doctor about the student's participation in the BiW4Teens program. Only use this form if you have obtained permission from the student's parent.
